THE MONACO CARDIO-THORACIC CENTER




Opened in 1987, the Monaco Cardio-thoracic Center is an independent medico-surgical institute, dedicated to the treatment of all cardio-thoracic and vascular pathologies. 

Diagnosis of cardiac disorders, treatment of emergency cases and the daily functioning of the Center are performed by a complete permanent medico-surgical team with 3 cardiologists, 3 anaesthesiologists-intensivists, and 3 cardio-vascular surgeons. (* see detailed list at the end).

This small hospital is open 24 hours a day, 365 days a year, and does not apply any selection procedure whatsoever with respect to its patients. All emergency and serious thoracic and cardio-vascular pathology cases are accepted, regardless of medical severity and the patient’s age or origin or social security cover.

Moreover, the clinic is a special medical entity, since in the same establishment, with the same facilities and technical floors, and a modular hospitalisation sector, patients of all origins and nationalities (most of them originating from the Mediterranean Basin) can benefit from treatment by world-renowned specialists from European and Northern American medico-surgical teams.  As soon as these specialists have received the approval of the Scientific Council, that approval is ratified by the Principality’s authorities and The Council of the Order of Doctors, enabling them to exercise officially in the Principality of Monaco. This “external” activity represents roughly 20% of the total volume of cases.


The Centre’s capacity can be varied around a base of 23 rooms, 47 beds, 7 day-hospitalisation units and 12 ICU beds. 
The 4th floor is a functional technical floor, accessed directly from the hospitalisation area, comprising, on the same level:

 

Two operating theatres

Two catheterisation rooms for invasive exploration and interventional cardiology. Since 1989, one of the rooms is an operating theatre as well as an angiography room, enabling the medical team to carry out exploratory procedures and, if necessary, endoluminal or surgical treatment, without having to move the patient. It was completely renovated in February 2007 with the installation of a Siemens AXIOM Artis dTA room.

The intensive care unit (12 beds), for medical, surgical, adult or paediatric emergencies or urgent cases, has direct access from the operating theatres and hemodynamics lab.


A non-invasive cardiology sector and day-care unit, on the 3rd floor, includes:

All the imaging equipment (particularly the magnetic resonance and multi-detector scanners) are run by physicians of the permanent cardio-vascular team and are at the permanent and exclusive disposal of cardio-thoracic and vascular pathologies, meaning that there is no delay between the need for an examination and the examination itself. It also assists clinical research focusing on modelling of the post infarct scarred heart (Gadolinium Late Enhancement Cartography).

 

A cardio-vascular exploration room equipped with Siemens Sonata 1.5 Tesla magnetic resonance imaging, since 2002,

A SOMATOM Definition : first dual source CT scanner, offering superb image quality, details, and low patient exposure, thanks to the fast image acquisition allowed by use of two X-ray sources and two detectors. This simplifies acute care diagnosis for any kind of patient, without use of Beta blockers. In March 2007, it replaced the 64-slice scanner bought two years earlier.



An ultrasound scanning department, equipped with two iE33 intelligent echo systems. 2 D and volumetric Live 3D imaging and quantification tools, helps to provide fast answers to the usual questions related to cardiac diseases : structure, efficiency, size and function. Stress tests are performed on an Easy Stress table.

Since the opening of the Monaco Cardio-thoracic Center in 1987, the number of patients hospitalised has exceeded 40 000. More than 45 000 interventional cardiology procedures (for diagnosis or treatment) and
16 000 surgical procedures have been carried out.

An additional original feature of the Monaco Cardio-thoracic Center is its billing system, based on a package price for each diagnostic or treatment procedure, established prior to starting the procedure.  

This package price covers all expenses, including hospitalisation and medical fees, laboratory and medical imagery tests, carried out in the Center or outside and the treatment of all postoperative unexpected or possible complications connected to the procedure, and is not related to the length of the stay.

This type of system, which gives a sense of responsibility to both the medical staff and the establishment, offers the best possible guarantee in terms of treatment quality.

 

The Monaco Cardio-thoracic Centre is more specially known and sollicited :

For paediatric activity, under the responsibility of F. BOURLON (cardiology), J. QUAEGEBEUR and Y. LECOMPTE (surgery) and F. MONTIGLIO (anaesthesia).

  • All types of percutaneous endoluminal treatment therapy (shunt and stenosis), extensive experience in ASD, PFO above 600.
  • For congenital diseases, very severe cases are referred from European countries and children are specially referred for the “Ross technique” (pulmonary valve auto-transplantation in small children, more than 100 cases operated on). In addition, each year, the Monaco Cardio-thoracic Centre welcomes numerous humanitarian cases from Eastern Europe and Africa.

For adult cardiology :

  • J. MARCO, M. SABATIER and their collaborators have a very long and in-depth experience in all types of endoluminal coronary intervention.
  • C. MIALHE is one of the most widely experienced practitioners of abdominal and thoracic aortic stenting (over 300 cases).
  • The cardio-surgical team has been called on for more than 20 years for the surgical treatment of post myocardial infarct severe LV remodelling, by the technique of the Endo Ventricular Circular Patch Plasty (more than 1 300 cases operated on) ; this Left Ventricle Reconstruction (L.V.R.) challenges indications for heart transplantation for severe ischemic CHF.

The vast majority (around 90%) of all extra-corporal circulation for adult and paediatric surgical procedures is accomplished without use of blood and blood products.
